An application is necessary for Cedar Lake services. Informational meetings and tours are available upon request. Additional documents that are required to accompany the application for waiting list include:

1. Guardianship Papers
2. Current Photo
3. Medical History
4. Dental History
5. Psychological
6. School / Workshop Records
7. Supportive Therapy Evaluations (OT | PT | SP | PSY | Neuro | Other)
